At Reliance Health, we work toward enhancing health through mental wellness, but we can’t do it without you. So much of the work we do depends on your support. Charter Oak is offering an easy way to double your contribution to our mission. From August 15th to November 23rd, Charter Oak will match donations made from their members to non-profit agencies, such as Reliance Health. The process is simple, just click here to download the form needed. You complete the top half of the form and mail it to Reliance Health, along with your donation. We’ll complete the rest and send it back to Charter Oak. Once they receive the completed form they’ll mail us a check to match any member gift of $25, $50, $75 or $100.
