Individuals receive our services become members. To connect with Reliance Health services, or for questions about eligibility, please call our Intake Department (860) 887-6536 ext. 508.
When a person meets with our Intake Liaison or a Clinician, they will be asked questions regarding their mental health needs, treatment history, and Reliance Health eligibility criteria.
If member criteria is met…
… a face-to-face or virtual meeting will occur with a caring profession to further discuss our programs and services.
If we are not the appropriate agency for support…
… staff will provide information about other treatment resources in the community.
To Get an Intake
If you are a provider who would like to refer an individual to our services, please fill out the following:
If you are seeking services for yourself or a loved one:
- Call our Intake Department (860) 887-6536 ext. 508
- Fill out the Intake Referral Form
- Have a copy of your ID, Social Security Card, and Insurance cards ready
Completed forms may also be either dropped off at our Morosky Building (2 Cliff Street, Norwich, CT) or faxed to our staff at (860) 887-2784.
